Every major breach investigation tells a version of the same story. Initial access is gained through a phishing email, an unpatched vulnerability, or a compromised credential. Then the attacker moves laterally for days or weeks, searching for privilege. When they find it — a domain admin account with a weak password, a cloud root key sitting in a script, a service account with permissions nobody audited — the breach escalates from a contained incident to an organizational crisis.
Privileged accounts are the force multipliers that turn a foothold into a catastrophe. And in most environments, they are far more numerous, far less controlled, and far more accessible to attackers than security teams realize. Privileged Access Management is not a compliance framework or a checkbox program — it is the set of controls that determines whether a breach stays small or becomes a disaster.
Defining Privileged Access in Your Environment
The first challenge most organizations face is simply knowing what privileged access exists. The obvious accounts — domain administrators, local administrators, cloud root accounts — are usually documented. The dangerous ones are often not.
Privileged access exists anywhere a credential, token, key, or session can take an action that bypasses normal authorization controls or grants access to sensitive systems. That definition encompasses more than most PAM programs acknowledge: database administrator accounts, application accounts with elevated permissions, service accounts used for batch jobs, API keys with write access to production infrastructure, accounts with privileged access to backup systems, and accounts that can modify security tooling configurations. In a mature organization, a privileged access inventory commonly reveals two to five times as many privileged accounts as the security team initially estimated.
Service accounts deserve particular attention. They are often created for specific purposes, granted broad permissions to ensure the application works, and then never revisited. Service account passwords may not rotate for years. The account may outlive the application it was created for. And because service accounts are often excluded from privileged access management programs — they do not belong to a person, so they fall into a gap — they accumulate risk quietly until an attacker finds them.
Cloud identity surfaces have added another category: roles and policies that grant privileged access without looking like a “privileged account” in the traditional sense. An AWS IAM role attached to an EC2 instance may have permissions equivalent to a domain admin in terms of the damage it can enable, but it may not appear in any privileged account inventory. Cloud PAM is a distinct discipline that many traditional PAM programs have not caught up to.
Credential Vaulting: The Foundation Layer
Credential vaulting — storing privileged credentials in a centralized, access-controlled, audited vault rather than in spreadsheets, wikis, personal password managers, or hardcoded in scripts — is the foundational PAM control. It is also the one that organizations most often implement poorly.
A credential vault provides two things: access control over who can retrieve privileged credentials, and audit logging of every access event. Without vaulting, privileged credentials are effectively shared secrets with weak accountability. When a breach occurs, you cannot answer the question “who had access to this account and when” because the credential was stored in a shared team password manager or a Wiki page accessible to half the IT department.
Effective vaulting programs also automate credential rotation. The vault should change privileged account passwords on a defined schedule — typically every 24 hours for highly privileged accounts — and in response to credential access events. When an administrator checks out a password to perform a task, the password should rotate after the session ends. This eliminates the risk of credential reuse if the password was captured during the session.
The implementation challenges are real. Applications and scripts often have privileged credentials hardcoded or stored in configuration files. Rotating those credentials breaks things, which is why they were never rotated in the first place. A realistic vaulting rollout requires an inventory phase, a dependency mapping phase where you identify everything that uses each credential, and an integration phase where applications are updated to retrieve credentials from the vault dynamically. This is painstaking work, but it is not optional if you want meaningful credential control.
Just-in-Time Access: Reducing Standing Privilege
Standing privilege — accounts that maintain administrative access continuously, whether or not they are being actively used for administrative tasks — is one of the highest-risk configurations in any environment. An account with permanent domain admin rights is a target every hour of every day, not just when an administrator is actively using it.
Just-in-time (JIT) access flips the model. Administrators request elevated access for a specific task, the access is granted for a defined time window with appropriate approvals, and the access expires automatically when the window closes. Between administrative tasks, the account has no privileged access. An attacker who compromises the account outside of an active privilege window gets ordinary user access, not administrative access.
Microsoft’s Privileged Identity Management in Entra ID is one mature implementation of this model for Azure and Microsoft 365 environments. CyberArk, BeyondTrust, and Delinea offer similar capabilities for hybrid and on-premises environments. The specific tooling matters less than the architectural principle: privileged access should be ephemeral, task-scoped, and approved.
JIT access also creates a natural audit trail. Every privileged session is tied to a specific request, a specific business justification, and a specific time window. Reviewing privileged activity becomes straightforward because every privileged action was preceded by a deliberate access request rather than being indistinguishable from ambient standing access.
The cultural friction is predictable. Administrators who are accustomed to always-on privileged access find JIT access inconvenient, especially initially. Managing that friction requires clear communication about why the change is happening, training on the new workflow, and making the JIT request process as smooth as possible. The inconvenience is real but manageable; the risk reduction is significant.
Privileged Session Monitoring and Recording
Knowing who accessed a privileged account is necessary but not sufficient. You also need to know what they did with that access. Privileged session monitoring — recording administrative sessions and making those recordings searchable and auditable — is the control that closes that gap.
Session recording serves multiple purposes. It creates an audit trail for compliance and forensics. It deters insider misuse, because administrators who know their sessions are recorded make different decisions. And it enables rapid investigation when something goes wrong — rather than attempting to reconstruct administrative actions from sparse system logs, investigators can review a recording of exactly what happened.
Modern privileged session management systems go beyond recording to provide real-time monitoring and intervention capabilities. You can define policies that alert on specific commands or actions — a database administrator running a DROP TABLE command, an administrator exporting a large user list, an unexpected process being launched during an administrative session. Some platforms allow real-time session termination when policy violations are detected, providing a control that can stop a malicious insider or compromised account mid-action.
Session proxy architectures, where administrative sessions are routed through the PAM platform rather than connecting directly to target systems, eliminate the need for administrators to know system credentials at all. The administrator authenticates to the PAM platform, selects the target system, and a session is established on their behalf. The actual credential is never exposed to the administrator’s workstation, which removes a significant class of credential theft risk.
PAM for Active Directory and Cloud Environments
Active Directory environments require specific PAM attention because AD privilege is effectively the master key to everything connected to it. We cover AD-specific risks in depth in our Active Directory Security Assessment Deep Dive, but from a PAM perspective the priorities are eliminating standing Domain Admin memberships, implementing tiered administration models that separate administrative domains, and ensuring that all AD administrative activity routes through your PAM platform.
The tiered administration model — Tier 0 for domain controllers and AD itself, Tier 1 for servers, Tier 2 for workstations and user devices — is the architectural foundation for AD privilege control. Accounts at each tier should only authenticate to systems at the same tier or higher, and privileged accounts should never be used for ordinary work tasks. A domain admin account that is also used to read email is a single phishing email away from domain compromise.
Cloud PAM requires different tooling but the same principles. Cloud provider IAM roles should follow least privilege, standing access to production environments should be replaced with JIT role assumption, and all privileged cloud actions should be logged to a tamper-evident audit trail. Cloud PAM platforms like Ermetic, Sonrai, or CIEM features within broader cloud security platforms can automate privilege analysis and right-sizing across AWS, Azure, and GCP.
Building a Sustainable PAM Program
PAM programs fail most often not in the initial implementation but in the follow-through. A vault that is deployed but not maintained, JIT workflows that administrators find ways to bypass, session recordings that are never reviewed — these represent significant investment with limited security return.
Sustainability requires treating PAM as an operational program, not an implementation project. That means quarterly privileged access reviews to identify accounts that should be removed or reduced, automated alerting when privileged accounts are created outside the PAM workflow, regular audits of vault access logs for unusual patterns, and integration between the PAM platform and your identity governance program so that privileged access is revoked when employees change roles or leave the organization.
The metrics that indicate a healthy PAM program: percentage of privileged accounts under vault control, mean time to provision and deprovision privileged access, coverage of privileged session recording, and the ratio of standing to just-in-time privileged access. Track those numbers over time and they will tell you whether your program is maturing or decaying.
Privileged access management is hard work, and the organizational change it requires is significant. But attackers have made their priorities clear — in breach after breach, privileged account compromise is the turning point. Organizations that control privileged access effectively constrain the blast radius of every other security incident. The investment is worth it.
